Candidate should send their filled in application form by Regd.Post with Acknowledgement due to “ The Deputy Director, Distance Education, D.B.H.P.Sabha (Kerala), Chittoor Road, Ernakulam, Kochi-682016 with the original certificates, mark sheets with one set of Xerox copies attested by a Gazetted Officer on or before the last date or in person . The last date for submission filled in the form is 15.5.2009
